Understanding the Impact of PCOS on Infertility

Posted on September 10, 2024

Polycystic Ovary Syndrome (PCOS) is one of the most common hormonal disorders affecting women of reproductive age, yet it remains widely misunderstood. In honor of PCOS Awareness Month, we at Reproductive Biology Associates (RBA) want to shed light on how PCOS impacts infertility and provide insights on how affected individuals can navigate their fertility journey.

What is PCOS?

PCOS is a complex endocrine disorder characterized by an imbalance of reproductive hormones. This imbalance can cause various symptoms, including irregular menstrual cycles, excessive hair growth, acne, and obesity. One of the most significant challenges women with PCOS face is infertility.

How Does PCOS Affect Fertility?

Irregular Ovulation:

Women with PCOS often experience irregular or absent ovulation, making it difficult to predict fertile windows. Without regular ovulation, releasing a mature egg for fertilization becomes inconsistent, significantly impacting the ability to conceive naturally.

Hormonal Imbalance:

Elevated levels of androgens (male hormones) are common in women with PCOS. This hormonal imbalance can interfere with the development and release of eggs from the ovaries, leading to the formation of cysts.

Insulin Resistance:

Many women with PCOS have insulin resistance, which can lead to higher insulin levels in the body. This condition can further disrupt hormonal balance and ovulation, contributing to fertility challenges.

Endometrial Health:

The irregular menstrual cycles associated with PCOS can affect the endometrium (the lining of the uterus), potentially making it less receptive to embryo implantation.

Diagnosing and Managing PCOS for Better Fertility Outcomes

Medical Diagnosis:

A diagnosis of PCOS is typically made based on a combination of medical history, physical examination, blood tests to measure hormone levels, and ultrasound imaging to observe the ovaries.

Lifestyle Changes:

Weight management through a balanced diet and regular exercise can help regulate menstrual cycles and improve insulin sensitivity. Even a modest weight loss of 5-10% can significantly enhance fertility in women with PCOS.

Medications:

Fertility medications such as Clomiphene Citrate or Letrozole are often prescribed to induce ovulation in women with PCOS. In some cases, Metformin, a medication used to treat insulin resistance, may also be recommended.

Assisted Reproductive Technologies (ART):

For women who do not respond to lifestyle changes and medications, ART, such as in vitro fertilization (IVF), can be an effective option. IVF involves stimulating the ovaries to produce multiple eggs, retrieving the eggs, fertilizing them in the lab, and transferring the resulting embryos to the uterus.
